1. Information received at 02:35 hrs. on 16.04.14 that 15666 Dn B.G. Exp. met with an accident between AJURI-JID at

Km 72/0-2-71/7 at about 02:09 hrs. where Loco No. 18689 WBM2 (NGC) along with 9thcoaches got derailed. No

casualty except one grievous injury, 10 simple and 47 trivial injure.

1. Derailment of wagon No.NE BCL - 63938 of Dn FCA Bhim in between LGT-MPP at Km49/2-49/8 at about 13:10 of

11.06.14Brief description: - Information received at about 13:45 hrs. that Wagon No. NE BCL - 6393814th from T/Engine

of Dn FCA Bhim, Loco No. 6497/6342 YDM-4 derailed & dragged about 600 mts. by rear trolley all wheel, at Km 49/2-49/8

in between LGT-MPPat about 13:10 hrs. of 11.06.14. Loco pilot goods of the train was Sri S. Das III/LMG, ALP - Sri P.

Sonowal/LMG and Guard Sri M.S. Narayan/LMG. Re-railed at: 21:15 hrs. T.C. given at 22:30 hrs. of 11.06.14 by

SSE/P-Way/MBG P.FCause: - Due to missing of wagon parts like plum brass bearing, locking plate and bogie coil spring,

given by Sr. DSO/LMG at site.


2. Derailment of wagon No. NR BCL -18183 of Up BHRB goods,hauled by Loco No. 6411 YDM4 at Km-5/6-7 in between

PNGM-KTX at about 05:07 on 16.06.14.Information received at about 05:22 hrs that wagon No. NR BCL -18183 of Up

BHRB goods., hauled by Loco No. 6411 YDM4 derailed & dragged about 15 mts. by all wheel at Km-5/6-7 in between

PNGM-KTX at about 05:07 on 16.06.14. The LP of the train was Sri C. M. Musahari/BPB, ALP Sri R. K. Mandal/BPB and

Guard Sri A. Kumar/BPB. Re-railed at: 10:40 hrs. T.C. given at 10:40 hrs. of 16.06.14by SSE/P-Way/BPB.P.F. Cause:

Under investigation.


3. Derailment of Dn FCA Bhim, Loco No. 6390 + 6354 YDM4, entered in to Catch siding of HJO station at 17:10 hrs. on

07.07.2014. information received at about 17:20 hrs that Dn FCA Bhim, Loco No. 6390 + 6354 YDM4, entered in to catch

siding of HJO station at 17:10 hrs. on 07.07.2014 resulting derailment of Loco 6390 YDM4 by all wheel and 6354 YDM4 by

leading trolley one pair wheel. Rear portion 18+1 wagons withdrawn clearing main line at 20:35 hrs. Remaining 7 wagons

also withdrawn at 21:20 hrs. and restored normal train running. No passenger train affected for the incident. The LP of the

train was Sri B. K. Das-II/BPB, ALP Sri A. Kumar/BPB and Guard Sri Pankaj Sharma/BPB.Re-railedat: 03:30 hrs. (Rear

Loco 6354 YDM4 and Front Loco to be rerailed after proper planning lateron).TC given at 22:05 hrs. by SSE/P-Way/HJO

dated: 07.7.2014.


P.F. Cause: Given by ADEN/HGS as Home Signal of HJO station passing at danger at higher speed.


4. Derailment of 15691 Dn Cachar Express, Loco No. 6489 YDM4 at KM 182/7-8 between SQF-BPB at about 06:35 hrs. on

08.7.2014.Information received at about 07:00 hrs that 15691 Dn Cachar Express, Loco No. 6489 YDM4 met with an

accident at KM 182/7-8 between SQF-BPB at about 06:35 hrs. on 08.7.2014 resulting derailment of rear SLR No. 4769

front trolley two wheel and dragged 100 mtrs. No casualty or injury reported. Front portion with 12 coaches left site at 07:35

hrs and arrived BPB at 07:53 hrs. The LP of the train was Sri S. Ghosh/BPB, ALP Sri D. C. Mahawar/BPB and Guard Sri

Pradip Paul/BPB.Works started at 08:35 hrs. and rerailment completed at 09:48 hrs. TC given at 09:50 hrs. by

JE/P-Way/BPB vide No. BR/120/07/14, dated: 08.7.2014 with SD 10 Kmph UFA.P.F. Cause:Given by DEN-II/LMG as

Safety strap of primary Spring Coil No.7 of derailed trolley is missing.


5. Derailment of Dn FCA Bhim/120 in between HTL –LGT at Km 34/3-6 at about 03:00 hrs of 15.08.14. Information

received at about 03:45 hrs of 15.08.14 from ASM/LTG thatDn FCABhim/120 hauled by loco no- 6536/6177 with load 25/49

while passing in between HTL-LTG station at km 34/3-6, the train met with an accident, resulting derailment of SR BCL

39984 11 th from B/Van by all wheels. The train got parted on run and the front portion with 7 loads arrived LGT station at

03:40 hrs. The LP of the train- Sri D.C. Devnath/LMG, ALP- Sri B.Mitra/LMG and Guard- Sri B.B.Paul/LMG. Restoration

completed: 12:25 hrs.T.C. Given at: 14:20 hrs by JE/P-WAY/MBG.P.F.CAUSE: As per Sr.DSO/LMG derailment took place

due to gradual- mounting of wheel about 2 mtrs from rail table that means (outer-rail) drag out side the rail at left hand

curve. Though the exact point of mount is not clearly visible, it seems to be mounted from fish plate join.


6. Derailment of Loco No. 6643 WDM4 of 52568 Dn between BHRB-LLBR at Km 72/8-9 on 06.09.14. Information received

from BPB control as 06:10 hrs. to 06.09.14 that Loco No. 6643 YDM4 9long hood) of 52568 Dn passenger got derailed by

front trolley two wheels in between BHRB-LLBR at Km. 72/8-9. The LP of the train was Sri R.C. Mazumder/BPB, ALP Sri

Pappu Yadav/BPB and Guard Sri Rajib Ranjan/BPB. Re-railed at 11:05 hrs. TC given:- 11:35 hrs. dated. 06.09.14. P.F.

Cause:- As per Am & ADME/BPB the derailment occurred due to accumulation of slip earth sand from the adjacent hill on

LHS rail, because of heavy rainfall.


7. Derailment of wagon No. NR BCL- 18188 of Up LMG goods in between MGE-MXR atKm-92/9 about 23:57 hrs. on

07.09.14. Information received from 00:15hrs by ASM/MGE of 08.09.14 that Up LMG goods hauled by Loco No. 6582/6615

YDM4 met with an accident between MGE-MXR at at Km-92/9 about 23:45 hrs. on 07.09.14. Resulting derailment of NR

BCL- 18188 by 3ed from B/Van rear trolley one pair.The LP of the train was Sri S. C. Dey/LMG, ALP, Sri U. K. Das/LMG

and Guard, Sri C. M. Singh/LMG.Re-railed at: 07:55hrs.TC given at 08:00 hrs by SSE/P-Way/LFG Vide No-AB/451/09/14,

Dated: 08.09.14.P.F. Cause: - Due to missing of wagon parts side bearer guide bolt, arch plate etc. given bySr. DSO/LMG

at side.


8. Derailment of couch No. GS NFG-2239 of 52571 Up KXJ-DLCR Pass in between BRGM-DLCR at about 11:50 hrs. at

Km 22/8-9 on 08.09.14 Information received at 12:10hrs by BPB control of 08.09.14 that 52571 Up KXJ-DLCR passhauled

by Loco No. 6579 YDM4 met with an accident between BRGM-DLCR at about 11:50 hrs. at Km 22/8-on 08.09.14.

Resulting derailment of couch No. GS NFG-22396thfrom T/Engine by rear trolley one wheel.The LP of the train was Sri

Abdul Zalil/BPB, ALP, Sri P. Kumar /BPB and Guard, Sri G. Roy/BPB. Re-railed at: 15:55 hrs. by MFD TC given at 16:00

hrs by SSE/P-Way/BRGM Vide No-AKD/105/09/14, Dated: 08.09.14.P.F. Cause: - Water logging on track and pumping

over LHS due to rain cause of derailment given by AM/BPB at side.


9. Derailment of wagon No. NE BCL- 62981 of Dn DMR Bhim goods inbetween DJA-PDJ atKm-80/3-5 about 12:30 hrs. on

12.09.14.Information received from 12:30 hrs of 08.09.14 thatDn DMR Bhim goods hauled by Loco No. 6177/6640 YDM4

met with an accident between DJA-PDJ atKm-80/3-5 about 12:30 hrs. on 12.09.14.. Resulting derailment of NE BCL-

62981 by 5thfrom B/Van front trolley two wheel with dragging 150 mts. approximately.The LP of the train was Sri M. S.

Biswas/LMG, ALP, Sri J. C Medhi/LMG and Guard, Sri B. C. Dey II/LMG. Re-railed at: 17:26 hrs.TC given at 17:30 hrs by

SSE/P-Way/MXR Vide No-RJK/393/9/14, Dated: 12.09.14.(10 Kmph Non-stop.)P.F. Cause: - Under investigation.


10. Derailment of wagon No. NE BCL- 63426 of Up LMG goods in between LJO-JTG at Km-121/3-4 about 21:10 hrs. on

13.09.14. Information received from 21:17 hrs of 13.09.14 thatUp LMG goods hauled by Loco No. 6391 YDM4 met with an

accident over bridge No. 398 in between LJO-JTG at Km-121/3-4 about 21:10 hrs. on 13.09.14. Resulting derailment of NE

BCL- 63426 10thfrom T/Engine by rear trolley all wheel.The LP of the train was Sri D. Sarkar/BPB, ALP, Sri P.

Acharjee/BPB and Guard, Sri P. Chakraborty/BPB. Re-railed at: 07:30 hrs.TC given at 07:30 hrs by SSE/P-Way/Con/HJO

P.F. Cause: - Under investigation.


11. Derailment of Loco No. 6482YDM4 of 52578 Dn SCL-DMR pass in between ARCL-KTX at Km-19/7-6 about 18:15 hrs.

on 17.09.14.Information received from 18:25 hrs of 17.09.14 thatDn SCL-DMR pass hauled by Loco No. 6482 YDM4 met

with an accident in between ARCL-KTX at Km-19/7-6 about 18:15 hrs. on 17.09.14. Resulting derailment of Loco No.

6482YDM4 by front trolley two wheels. The LP of the train was Sri B. Rabha/BPB, ALP, Sri P. Yadev/BPB and Guard, Sri

R. Ranjan/BPB.Re-railed at: 21:45 hrs. by MFD TC given at 21:50 hrs by SSE/P-Way/SCL Dated: 17.09.14.P.F. Cause: -

Due to obstruction by ballast (size 50 mm.) place on rail table about 2 mts. length onboth rail (at Km 19/9) by unknown

person given by ADEN/BPB at side.


12. Derailment of wagon No. NE BCL-63623 of Dn FCA Bhim-142 in between DJA-PDJ atKm-80/1-4 about 01:55 hrs. on

18.09.14. Information received from 02:15 hrs of 18.09.14 that Dn FCA Bhim-142 hauled by Loco No. 6641/6661 YDM4

met with an accident between DJA-PDJ atKm-80/1-4 about 01:55 hrs. on 18.09.14.. Resulting derailment of NE BCL-63623

by 3ed from B/Van all wheel.The LP of the train was Sri K. C. Naha/LMG, ALP, Sri A. Raj/LMG and Guard, Sri K. M.

Sarkar/LMG. Re-railed at: 09:35hrs.TC given at 10:35 hrs by SSE/P-Way/MXR, Dated: 18.09.14.P.F. Cause: - Due to

mountingof wheel on rail table gradually about 2.5 mts. & drop out side rail at curve on Br. No. 178. During observation of

wagon one top side bearer of R/H (No.3) missing, which could not be traced out at effected and non effected portion.Given

by Sr. DSO/LMG at side.


13. Derailment of coach No- NF GS 2433 & NFGS 2325 of 52564 Dn at KXJ yard in L/2 on22.09.14.Information received at

10:00 hrs that 52564 Dn SCL-AGTL passenger hauled by No-6484, with 14/18 load met with an accident at 09:55 hrs while

entering KXJ station on L/1. As a result of which coach No- NF GS 2433, 5th from T/E got derailed by rear trolley all wheel

& NFGS 2325 6th from T/E got derailed by front trolley all wheel blocking main line between KXJ-BXG and KXJ-NLBR also

dragging about 100 meters. Point No-18 got damaged. The LP of the train was P. Dostidar/BPB, ALP- P.K. Nath/BPB and

Guard- K.R. Deb/BPB. Re-railed at: 16:35 hrs. TC given at 20:10 hrs by SSE/P-Way/KXJ vide No- SS/53/9/14, Dated:

22.09.14.

Achievement highlights:-


  1. There is no level crossing accident (manned & unmanned) during last three years. This is due to continuous counseling of gateman, LP, ALP, Guard etc. Road users of level crossing (Vehicle driver, two wheeler driver and pedestrians) are frequently counseled along with distribution of pamphlets in all level crossings (manned & unmanned) educating people about motor vehicle act UMLC Gate and how to cross manned level crossings.



  1. ARME staffs are made capable of evacuating trapped people coach within 10 mts. by cutting windrow bar and sheet through continuous mock practice.



  1. Alcoholism amongst running staff in the Division is drastically reduced through continuous Ambush checked at different locations other than Crew Lobbies.
